Overview
NHS - The National Health Service is the publicly funded healthcare system in England, and one of the four National Health Service systems in the United Kingdom.
We were contacted by an intermediary to design a simple mobile device App (IOS and Android) for the NHS, that helps patients log health metric readings regularly and share with their GP and/or other health professionals.

Visualising the user journey
After the initial rounds of interviews with the GP team, we created a user journey map. This map visualised the entire process — from patients becoming aware of the app through their GP to independently recording and sharing their health data.
This visual document helped all stakeholders align their understanding of the project scope and sparked discussions of possible pain points and opportunity areas to add value for the user.

Understanding the data
Through content modeling, we comprehensively mapped out health data relevant to the app's supported conditions. This approach allowed us to prioritize information effectively and present it in a user-friendly manner, avoiding information overload.

From App to Action - Defining the value proposition
Our conversations with medical professionals, specifically doctors, revealed that a simple diary-style app wouldn't suffice. Accurate diagnosis often requires patients to record readings for a minimum duration or at specific intervals, depending on the health condition.
The app had to deliver on 2 key aspects:-
Educating/ creating a mental note about the frequency of input required (daily/weekly/monthly and within the day-once, twice or thrice)

Reminder to input the readings (reminder could be once a day for all readings required for that day - or different reminders for every reading within a day - user to choose)
To be able to help with both of the above the platforms/system needs to know what is user's expected frequency of input (either for a prescribed period or for an ongoing period).

Orienting the user & the system
Diary set up- A basic schema (having certain defaults) for each health diary which helps orient both the user and the system.
Delivered by the idea of a 'Daily tray' - a quick access single interface to input all readings for the respective diaries. The Daily tray also helped inform/remind the user of the frequency expected.

Information Architecture
Just like a blueprint, the IA defines how content and features are organised within the app. This ensured users could find what they need quickly and intuitively. We roped int he dev leads at this point since the IA also laid the groundwork for the app development process.
